What is the Spanish health form (spth)?

Travelers entering Spain must complete the Health Check for Travel to Spain (SpTH application), which was created to reduce the spread of coronavirus among Spanish citizens and visitors.

Please also note that this health declaration does not replace a visa. If your nationality requires a visa to enter Spain, you will be asked to present it upon arrival in our country. We suggest that you contact your local Spanish embassy if you need a visa.

Who will be allowed to enter Spain with this document?

To protect their citizens from infectious pathogens, Spanish authorities require that all passengers arriving in Spain by air or sea (including those in transit to another country) for short visits complete a health check form and present a certificate of vaccination, proof of cure of COVID-19 (a codeine-based flu vaccine) or a negative test result for the virus.

Children under 12 years of age do not need to provide vaccination certificates to enter school, but must still complete the health check form.

The applicant will submit the following mandatory documents:

A negative COVID-19 PCR test is required before your departure.
A scan of your passport information is expected.
As well as a national ID card.

To complete the form you will need :

International transportation data (date of arrival, flight/ship number, and name, airline, seat/cabin number, etc.)
Information about your stay in Spain.
You must carry proof of vaccination, diagnostic test, or treatment for COVID-19 if you are traveling from a high-risk country.
Your email address.
Your identity document number (passport, permit, etc.)

To travel to Spain, do I need a health control form?

Yes, it is mandatory to have a health check form with a QR code. Each person must have their health check form. The QR code on the form is personal and not transferable.
